Dosage Guidelines: Start Slow, Stay Low

Methylene Blue Dosage Principles

Methylene Blue (MB) demonstrates what is known as a hormetic dose-response curve: its effects depend heavily on the dose. At low doses, MB acts as a mitochondrial and cognitive enhancer with antioxidant properties. However, at higher doses (above 4 mg per kg of body weight), it can become pro-oxidant and potentially toxic. Understanding this principle is crucial for safe and effective use.

A classic example is the treatment of methemoglobinemia, a blood disorder that impairs oxygen delivery to tissues. MB has been shown to be highly effective at low, weight-based doses (typically 1 mg/kg intravenously). However, at higher doses (above 4–7 mg/kg), it can, paradoxically, induce methemoglobinemia and other toxic effects.

With MB, more is not better. Research consistently shows that cognitive and
mitochondrial benefits occur at low doses, while higher doses increase health risks.

Administration Methods

MB is available in several forms, each with unique considerations:

Liquid Solutions

  • Typical concentration: 1% (equivalent to 10 mg/mL)

  • Dosing: Measured with a dropper or oral syringe

  • Usage: Can be diluted in water or taken directly

  • Considerations: Dosages are difficult to calculate (see below)
  • Note: MB can stain skin and mucous membranes; rinse mouth after use

Capsules/Tablets

  • Standard doses: Usually 5–25 mg per capsule

  • Advantages: No staining, convenient, standardised dosing

  • Considerations: May contain additional ingredients

The Dosage Challenge

Determining the correct dosage of Methylene Blue can be confusing. Some experts recommend dosing by body weight, while others suggest taking fixed daily amounts. The challenge is compounded by the fact that most MB products are sold as liquid solutions, which can make accurate dosing tricky.

  • Weight-based dosing is typically used for acute medical conditions (e.g., 1 mg/kg for methemoglobinemia).

  • Fixed-dose regimens (e.g. 10–40 mg/day) are common for supplemental use aiming at cognitive/mitochondrial benefits.

Calculating Liquid Dosage

  • 1% solution: 1 drop = 0.5 mg of MB. Thus, 20 drops = 10 mg.

  • 2% solution: 1 drop = 1 mg of MB. Thus, 10 drops = 10 mg.

Note: These are approximate values, as drop size and solution strength vary by brand.

Because of this variability, many experts recommend capsules or tablets for more precise dosing.

Summary

  • Low doses of MB offer mitochondrial and cognitive benefits.

  • High doses can be toxic and are reserved for specific medical emergencies under professional supervision.

  • Liquid solutions require careful measurement; capsules/tablets provide more reliable dosing.

  • Read the Safety Considerations and Side Effects risks below.
  • Start low, go slow, and never exceed recommended limits. Consult a healthcare professional for personalised advice.

Administration Tips

  • Taking Methylene Blue on an empty stomach may enhance absorption
  • Morning administration is often preferred due to potential mild stimulatory effects
  • Cycling usage (e.g. 5 days on, 2 days off) is recommended to prevent tolerance
  • Pharmaceutical-grade Methylene Blue is essential for safety.
  • There are strong synergies between Vitamin C and Methylene Blue and you can enhance the potency of Methylene Blue by taking it with some ascorbic acid. This is because ascorbic acid changes Methylene Blue into into leukoMethylene Blue which increases its antioxidant potential, thereby helping to reduce oxidative stress and cellular damage.

Safety Considerations

While Methylene Blue has a long history of medical use and is generally considered safe at appropriate doses, there are important safety considerations to be aware of:

WARNING: Methylene Blue is a MAO inhibitor (see explanation below) and should NEVER be used with serotonergic (anti-depressant) medications, including:

  • Selective Serotonin Reuptake Inhibitors (SSRIs)
  • Serotonin-Norepinephrine Reuptake Inhibitors (SNRIs)
  • Tricyclic Antidepressants
  • MAO Inhibitors
  • Certain pain medications
Monoamine oxidase inhibitors (MAOIs) are a type of medication primarily used to treat depression by blocking the action of an enzyme called monoamine oxidase, which is responsible for breaking down neurotransmitters such as serotonin, dopamine and norepinephrine. These neurotransmitters play a key role in regulating mood, and MAOIs allow them to remain active in the brain for longer periods.Combining Methylene Blue with any of the above (and that also includes sceletium) can result in dangerously high levels of serotonin circulating in the brain which can be extremely toxic. If you are taking any of these medications, you MUST consult with your health care provider before considering Methylene Blue.

Additional Precautions

  • Pregnancy and Lactation: Methylene Blue should NEVER be taken during pregnancy or while  breast-feeding unless prescribed by a doctor
  • G6PD Deficiency: Individuals with glucose-6-phosphate dehydrogenase deficiency should avoid Methylene Blue due to risk of hemolytic anemia
  • Kidney or Liver Disease: May require dose adjustments of Methylene Blue
  • Quality: Use only Pharmaceutical-grade Methylene Blue that has been laboratory-tested for heavy metals.

Potential Side Effects

Even at appropriate doses, Methylene Blue may cause certain side effects:

Common Side Effects

  • Blue discoloration of urine and stool
  • Temporary blue discoloration of tongue and lips (if taking liquid)
  • Mild gastrointestinal discomfort
  • Slight headache or dizziness (rare at low doses)

Less Common Side Effects

  • Increased blood pressure (typically at higher doses)
  • Anemia in susceptible individuals
  • Skin sensitivity to sunlight
  • Fatigue or cognitive effects if overdosed

Side effects are reportedly generally mild at the low doses typically used for cognitive or mitochondrial support. Discontinue use and consult a healthcare provider if you experience any concerning symptoms.

Purity Matters

Methylene Blue is available in various grades:

  • USP/Pharmaceutical Grade: The only grade appropriate for internal use in humans.
  • Laboratory Grade: Intended for research. May contain harmful impurities not suitable for human consumption.
  • Aquarium/Fish Tank Grade: Contains additives and impurities that can be harmful.

Always verify that any Methylene Blue product intended for internal use is USP (United States Pharmacopeia) or Pharmaceutical Grade. As a further precaution, you can request an independent laboratory report of heavy metal content from the supplier. Any reputable supplier should produce one on demand. As an example, see this page for a lab report and analysis of heavy metals in the Methylene Blue product supplied by Pharmatropia.

Medical Supervision

Using Methylene under the guidance of a healthcare provider familiar with its applications is strongly recommended, especially for those with existing health conditions or taking medications. Please read our Disclaimer.
